Disebabkan Doktor pelatih? Atau ke hospital swasta ternama yang
memang tiada doktor pelatih?

No room for ignorance in hospital wards
I AM a senior doctor who works in a government hospital with
house officers. Having read letters and comments by these house
officers and their parents regarding the harassment by their senior
colleagues, let me share some of my experiences.
> A middle-aged man, admitted to ward for severe diarrhoea, died
due to hypovolemic shock (life-threatening fluid loss). The reason:
The house officer on call did not check the patient抯 vital signs
documented in the record chart.
> A house officer during his morning rounds continued serving
potassium supplement even though the patient has hyperkalemia.
The reason: He did not review the patient抯 drug chart during
rounds. He did not trace results of blood tests taken two days ago.
> A young man was transferred from a district hospital for chest
pain. The doctor who first saw him had missed the ECG findings of
acute myocardial infarction (severe heart attack).
As a result, the man received delayed treatment, and ended up
with heart failure. The doctor was a house officer from my hospital,
sent out to the district hospital after having completed two years of house officer training.
> A man who collapsed in the ward at 7am was left unattended.
The house officer who was on call the night before had left the
ward to take a bath, even though he had been informed that the
patient was unstable. The patient died due to a delay in
administration of CPR.
These are some of the scenarios where the house officers got a
shelling from me, not only because of their severe deficiency in
knowledge, but due to their attitude and half-hearted manner
when dealing with patients.
Then there are house officers who habitually come late to work,
even later than their senior colleagues. And there are house officers who don抰 take blood investigations on time for patients with
dengue fever and never bother to trace the results on time,
putting patients at risk.
There are also house officers who smoke in the doctor抯 room and
house officers who go out drinking late into the night, and are
unable to come in to work the next morning due to a severe
hangover.
These are the house officers that I have scolded. They grew up,
finished their two years |

tgk nasib gak la..ade hospital gomen yg ok..ade yang x ok..pengalaman aku..
wife bersalin anak pertama-hospital kuala krai..doktor dan misi yang ok..banyak membantu isteri dan anak aku..Alhamdulillah..dua2 selamat..bayar pun 20 hengget jek..
aku masuk wad..hospital kuala pilah..aku kene denggi..duk wad 3 hari..misi+doktor ok jek..doktor pun muda lagi..sebaya aku gitu..cuma aku perasan la..ade doktor housemanship kene marah dengan doktor senior..takut dia nak amik darah aku..doktor tu udahnyer suruh misi amik darah aku..payment yilekkkk..
anak aku kene check darah..suspek denggi-hospital selayang..daftar x banyak karenah..lepas register dalam 10 minit gitu dah cek +amik darah..pastu dlm 10 minit dah tau result dan jumpa doktor balik untuk consultation..charge singgit jek..untuk registeration..so far ok.. |

aku buat pembedahan buang batu karang.. kat UMSC a.k.a.. hospital universiti ajer... ok jer... doctor punya layanan mmg baguih la... tapi klinik kerajaan dah kompom ramai orang...
btw... utk pakar... negera kita ni bukannya ramai sgt pakar... dan pakar2 yg limited ni sebenarnya bekerja utk byk klinik & hospital... jadi ada kemungkinan kita deal dgn doktor yg sama di hospital yg berbeza! |

medang..thanks dor the pinat..btw..macam hospital klang tu aku dengar bad record..slalu dengar citer negatif..aku rasa sebab crowded..ye la..yang Shah alam pun pi situ..klang confirm la pi situ..banyak sangat area dia cover..tu yang agaknyer doktor dan misi takleh nak bagi yang terbaik pada patient..penat.. |

sebab diorg tak nak reputasi hospital swasta terjejas kalu2 apa2 terjadi kat patient! |

sorry aku tak setuju ngan statement ni....
kadang-kadang... hospital swasta memang terpaksa refer kes berat pada hospital kerajaan... but provided dgn alasan kemampuan pesakit dan keluarga pesakit tu sendiri.
Baru jer pagi tadi aku kena komplen ngan relative patient sbb benda ni sebab diorang tak nak paham... tak pasal-pasal....
Masa buat admission, admission clerk dah cakap.... dia kena standby RM40k kalau nak admit (adik dia accident, cedera bahagian kepala) in case kena buat surgery dan in case jugak kalau kena buat surgery during emergency hour... Mungkin disebabkan explanation yang kureng dari doctor kat ER, mereka setuju untuk admit tanpa berfikir panjang... yerlah.... dah takot tgk darah semua..
Bila specialist datang review patient... dia explain kata sekarang tak payah buat operation, tapi kalau tiba-tiba patient deteriorate i.e. tak sedar diri, tak boleh bercakap... tak dapat gerak part of his body... dia akan buat surgery under emergency... then dia explain berapa cost.. so dr tu kasik figure dalam RM20k - 40k.... ini bukan la nak kata sekali charge.... dr tu pikir jugak bila case yang melibatkan major surgery... hospital care bukan setakat sehari dua.... mungkin berminggu... sesetengah case kadang melibatkan sebulan lebih...
Kalau ader insurans coverage.. memang la mudah dia nak proceed untuk treatment tapi kalau family takder duit... ini dah kira membebankan family pesakit sendiri... mana nak gagau duit...
Sudahnya.... baru masuk tak sampai setengah jam... (in fact specialist tu datang 5 minit lepas patient masuk unit aku pukol 2.30pagi dan dia tgh tidor masa diorang call refer case tu) diorang terus mintak transfer hospital lain. Benda-benda macam ni sedikit sebanyak tentulah membengangkan specialist tu... tapi sekurang-kurangnya aku tgk specialist tu tak marah diorang... cuma bila dia asik reinforce pasal duit, family tu dah tension.
Bila dr dah blah.... aku pulak yang jadik mangsa sebab kakak patient tu kata, orang bawah yang tipu dr kata diorang ada insurans, walhal aku dah tanya clerk kat bawah dan diorang memang dah cakap diorang takde insurans dan self-payment.... bila aku cakap aku akan call diorang untuk clarify, pompuan tu terus cakap "Eh... tak payah.. tak payah bising2...." masa tu aku tau dah... ni kes nak kaver line... lepas tu dia bising2 kata hospital ni penting kan duit la... dari masa dia jejak masuk semuanyer money talk katanya.....
Then dgn megahnyer dia cakap "Kalau I tau, since mula2 I bawak adik I pegi governement hospital!!!"
Yerlah tu..... silap2 adik kau terbongkang lagi ngan CT scan semua tak buat... dr pon tak dtg attend kalau kau pegi GH tu...
so, bila hospital swasta kata nak hantar pegi government hospital.... bukan sebab kes tu berat atau tidak.... ia disebabkan samaada mereka mampu atau tidak sebab kita sendiri tak nak membebankan ahli keluarga pesakit.... dan berapa ramai dr kat private hospital yang tak charge pesakit diorang sebab tau mereka tak mampu?? semua ni takder saper pon nak ambik tau... dan semata-mata sebab kita tak nampak.... saper kata kes berat takde dekat hospital swasta?
Dan bila baca luahan dr kat atas dalam 1st posting tu... memang berbeza giler treatment nyer ngan treatment kat tempat aku...
Bapak aku sakit jantung dimasukkan ke wad biasa kat GH ngan dr pelatih yang bengap tak gheti amek history!!! Last2 aku yang tanyakan soalan dgn lebih tepat kat bapak aku... Nurse bagi ubat tak pakai otak, blood pressure bapak aku dah low... still nak serve 2 jenis ubat yang boleh merendahkan lagi blood pressure nyer. Bila aku bukak mulot tanya tarik muka dan kata kena makan. Bila aku tanya tak tau ker BP dah low??? Tak tau ker side effect ubat??? Baru terhegeh2 nak tengok chart!! Bila aku mintak nak jumpa dr untuk refer case pegi spital lain, sikit pon diorang tak layan... sampai sudah bapak aku kata tak yah la.. jgn bising-bising....
In the end, aku mintak tolong dr kat spital aku contact Specialist hospital sana, untuk refer case kat IJN... the moment director sana mintak case notes bapak aku, barulah diorang layan bapak aku baik-baik sebab ingat bapak aku contact kuat kat sana... sedangkan nan ado.... aku sendiri tak kenal sesaper cuma hasil pertolongan dr kat tempat kerja aku jer.... sini pon dah nampak double standard....
Adeh... terpanjang lebar lak aku nih.... |

yg paling menginsafkan aku kalau pegi hospital kelang kat wad biasa diaorg, dah macam bombay hospital...ada katil yg diletak dekat balkoni hospital je, sbb tak cukup space. |

utk aku sndr pengalaman bersalin kat spital serdang bole kate bagus gak ler. Nurse yg garang xramai..ramai nurse muda belia..layanan sumer ok. sampai budak yg anto mknn hari2 pun agaknye dilatih.."jemput mkn puan, 1jam lg saya dtg amik".. mae tu aku masuk wad kelas 3 aje..
pastu anak aku kene jaundis..admitted kat spital tampin. sini pun bagus, nurse mmg baik2..xde plak yg garang2..pastu seme nurse kat sane mmg betul2 tlg aku yg 1st time dapat anak & diajar cara nak breastfeed baby..(sbb kat spitall ni xleh bwk botol/puting)
so far aku masih lagi ke klinik/spital gomen..xpelah kene tunggu lame sket..yelah, org pun ramai..
lagipun kementerian kesihatan malaysia antara yg terbaik di dunia.. |

pengalaman bersalinkat hosp gomen sungai buloh.. layanan okes jek.. nurse2 muda2.. mcm adik2 aku hehe.. mmg baik la takde la tengking2 ke ape.. sejuk jek ati ..
tapi berbeza plak dgn experinece kakak aku besalin 2x kat hospital gomen Taiping... nurse dia mak aii.. garang gilos.. pastu stok2 muka masam .. ni biasanya nurse yg dah tua yg perasan bagus.. hampess jek.. dah le org sakit pastu tengking2 buat kita cam budak.. actually terpulang pada hospital tu sendrik la.. tak semua gomen takbaik .. ikut nasib la.. |

iyup setuju certain houseman kekadang x geti buat keje...malah selalu bongkak esp kalau houseman tu baru seminggu atau dua minggu gradeuate n baru dpt posting....banyak kali i tgk..malah kalau kiter tanya pasal ubat ada yg kurang ajar ckp "sy doctor sy tau keje sy" nasib baik ler masa tu i x der kat situ..dia cuma ckp kat my mom ..kalau ada kat situ mmg silap besar ler dia. |

mungkin sbb hospital baru kot...tu yg dpt layanan yg baik..mmg x nafikan hospital baru yg punya facility yg lengkap n x ramai patient mungkin dpt layanan yg baik dari nurse n doctor sbb diorg tak tension..
tapi masa zaman sekolah2 (90-an) dulu i pernah masuk hospital klang...alhamdullilah masa tu nurse 2 ok x marah2..even doctor pun ok...layanan dorg baik..compared to or complaint pasal hospital klang skrg nie.... |
